I am in network with PEIA/UMR, Aetna, and Peak. Please note that what you will pay out of pocket depends on various factors, including whether or not you have a deductible to meet and if you have coinsurance. It is your responsibility to know your insurance benefits; however, I will try my best to help.
It is recommended that you call the number on the back of your insurance card and ask the following questions:
Clients with insurance who are opting out of using their insurance may reach out to receive a superbill, which is a detailed invoice with CPT codes that they may submit to their insurance company for reimbursement.
Self-pay rates are $220 for an intake and $200 for a 60 minute psychotherapy session. I provide limited sliding scale slots ranging between $125-$200.
Under law, health care providers are responsible for ensuring that clients who are not using insurance or are paying out of pocket are provided with a Good Faith Estimate. which is the total estimated cost for non-emergency services. You will be prompted to sign a payment authorization form and will need to upload your credit or debit card to the HIPAA compliant patient portal prior to beginning services. Please note that payments need to be made in full by the end of the month. If you need accessibility regarding payments, I encourage you to explore this with me and we will come up with a plan to best accommodate your needs.
